Community Health Worker (CHW) Program
The Community Health Worker (CHW) Program provides community-based support to help individuals and families navigate health, social, and resource-related challenges. CHWs serve as trusted liaisons between community members and health and social service systems, offering education, referrals, care coordination, and advocacy.
Services may include assistance with accessing healthcare, insurance navigation, chronic disease education, connection to food, housing, transportation, and other community resources, and follow-up support to reduce barriers to care.
